The Home Care Nursing Program is curative and rehabilitative or palliative. Health promotion, prevention and support are integral to the care or the client and his/her family. Home care contacts are with individual clients, who have a specific diagnosis, and with their families.
Clients are referred for service by a nurse, doctor, family of friends or agency. The nurse identifies the need for home care and refers to the Personal Support Worker/ Health Care Aide or Home Maker., doctor, other support services such as Physiotherapist, occupational Therapist, etc.
Primary case coordination and hands on intervention is a main focus of care. Surveillance and treatment of high risk acute illness and unstable chronic illness. More invasive type of treatment may be wound care/dressings, parenteral therapy, ostomy care, etc. Admission may also be for health status monitoring, or for disease specific teaching (e.g., diabetes, chronic pulmonary conditions or heart disease). Supervision, monitoring, and teaching clients about their medications is a regular responsibility.
Home Care staff assist the client to manage their illness through teaching, treatment, and monitoring, to enable the client to remain as independent as possible which allows the client to remain in their homes and near family and friends as long as possible. Sometimes this means proving palliative care when the client chooses to remain at home during the last days of their lives.
