The S.L.I.C.K. visit will provide: Screening for Limbs, I-Eyes,
Cardiovascular, and Kidney.

• Provided by the Bigstone Health Commission to all people with Diabetes in the community.

• To provide access to Diabetes care and education.

• Teaching on cholesterol, high & low blood sugar levels, nutrition, foot care, prevention, active living

• Referrals & practice of confidentiality

• Blood pressure check:
• Height, weight and waist circumference check to assess your risk of heart disease
• Foot Assessment
• Urine check to assess kidney function
• Blood test to assess blood sugar control and cholesterol levels
• Retinal photographer will be available quarterly/year.
  
  If you have diabetes and you have not been checked by a doctor in over a year, this is your opportunity to find out if there are any diabetes related complications that you should be aware of.
